Step 1 –   Gather DOCUMENTS - All are required

• Birth Certificate
• Social Security Card of student
• Immunization Records
• Final Report Card from previous School and/or Withdrawal Paperwork
• Lease or Deed or Mortgage Statement 
• Current Utility Bill (Gas, Water or Electric)
• A government issued photo ID of parent/guardian showing current FBISD address

          NOTE:  ALL residence documents must show the address zoned to Fort Settlement Middle School and be in Guardians Name. 

Step 2 – Complete ONLINE REGISTRATION at https://www.fortbendisd.com/newstudent

Step 3 –  PROVIDE documents via email to FSMS Registration

  Please provide all documents in one email as separate attachments

                    NOTE:  Parent/legal guardian must provide student's documents.
