Your student will be sent home with the white copy of the 'Teacher's Referral to Nurse' when they come to the clinic. In this form you will find the time the student came to the clinic, the time they left, the reason for the clinic visit, what was found on the exam and the treatment the student received. The clinic sends this form home to keep parents informed of their student's visit to the clinic.
