    Clinic Release

    Please check in at the front office to pick up your student at the clinic.
     
    When returning to school after an absence, the student must give a note from the parent/guardian explaining the absence to the attendance clerk.
    Students with a contagious disease requiring a doctor's release to return to school should report to the clinic before going to class.  
     
     

    The FBISD School Health Services program is dedicated to creating a culture of wellness that supports student success.

    A Registered Nurse is assigned to each elementary and middle school campus, while a Registered Nurse and Licensed Vocational Nurse are assigned to each high school campus. The school nurse assists students' physical, mental and social growth by providing a health services program that assesses, evaluates and improves the health of students and staff.

    Please contact your school nurse regarding your student's health issues and concerns, hospitalizations and/or treatment changes.

    Please remember to put an extra set of clothing in their backpack in case of a personal accident.

  • Your student will be sent home with the white copy of the 'Teacher's Referral to Nurse' when they come to the clinic. In this form you will find the time the student came to the clinic, the time they left, the reason for the clinic visit, what was found on the exam and the treatment the student received. The clinic sends this form home to keep parents informed of their student's visit to the clinic.

  • Every school year, Kindergarten, First, Third and Fifth grades have hearing and vision screenings. Should your child brings home a referral form, please take this form to your health care provider to examine your child and then return the completed form to the school nurse.

  • Fort Bend ISD was not selected by Eye Care for Kids to be part of the Project See to Succeed. Should your child receive a referral to get an eye exam, please have the doctor complete the paperwork and return to the school nurse.

    Eye Glasses

     

    If your student loses their glasses, please check with the front office. At the end of the school year, any unclaimed glasses will be donated to a worthy cause.
