AFJROTC Volunteer Form

This is for cadets volunteering for events with other organizations. Complete the form and return to C/Huang for verification.

This Department of Defense form is mandatory for students who want to participate in AFJROTC. Each student and parent/guardian must complete and sign the form and return it within TEN school days of school starting or entering the program. Cadets who do not return the form are subject to disenrollment from AFJROTC.

This Department of Defense form is mandatory for students who want to participate in JROTC. Each student and parent/guardian must complete and sign the form and return it within TEN school days of school starting or entering the program. Cadets who do not return the form are subject to disenrollment from JROTC. 

A mandatory form required of all cadets to participate in AFJROTC. It describes how data is maintained in the AF/SF JROTC database. Additionally, the Health Screening Questionnaire affirms the permission given on the DD Form 3203 JROTC Code of Conduct and Parent/Guardian Consent Form. It identifies any health-related conditions that the AF/SF JROTC instructors should monitor.
