Withdrawal Process:

1. Fill out and sign an INTENT TO WITHDRAW form below.
2. Communicate with Mrs. Blackmon-Clark via email at wanda.blackmonclark@fortbendisd.gov, of your students withdrawal.  
3. Return the INTENT TO WITHDRAW form along with a photo of Guardian DL/ID to Mrs. Wanda Blackmon-Clark via email.

Upon completing these steps, the student will then be withdrawn from Christa McAuliffe Middle School.
